Online Purchase Requisitions information and instructions. Please review these instructions before using the Online Purchase Requisitions form. Some web browsers may need an additional plugin to access this form. Safari web browser Safari users should be able to use the form without additional plugins. Use this link to access the online Purchase Requisitions web page. Internet Explorer web browser Internet Explorer users should be able to use the form without additional plugins. Use this link to access the online Purchase Requisitions web page. Firefox web browser Firefox users will need to install the fx-quartz.pdf plugin. Please follow these instructions to download the plugin.

Step 1	Go to the following web page: http://forms.lusd.net/purchasing/purchaseRequisition.html
Step 2	The following dialog box will appear. Click on the Cancel button (as indicated below).
Step 3	Click on the Install this plugin link to install the needed Plugin (as indicated below).
Step 4	A yellow caution notice will appear. Click on the Allow button (as indicated below).
Step 5	A dialog box will appear. Click on the Install Now button (as indicated below).
Step 6	Once the installation is complete another dialog box will appear. Click on the Restart Firefox button.
When Firefox restarts you will automatically be taken back to the Purchase Requisition form where you can begin entering information. You will not need to install this plugin again. The next time you use the Online Purchase Requisition form it will come up in the Firefox web browser.
